Sweetwater Medical’s Disease Management Program gives physicians peace of mind knowing that their patients are receiving the finest quality homecare. Our in-house pharmacy dispenses the respiratory medication directly to the patient. A once-a-month callback with the disease management program insures patient compliance. Any change in the patient status is communicated to the physician by our respiratory therapist.


Therapy for Sleep Apnea
CPAP/Bilevel therapy to include: patient evaluation, education, and clinical monitoring. Our Disease Management Program is structured to insure high patient compliance and encourage our patients to participate in their rehabilitation. Home
visits and call-backs are performed by our respiratory therapist.


Therapy for COPD/CHF
All of our Disease Management Programs are overseen by a licensed respiratory therapist. Oxygen therapy is administered by our trained technicians. To supplement our superior service, Sweetwater Medical has state-of-the-art products and equipment including lightweight portable oxygen systems that promote an active lifestyle. Any noncompliance issues are communicated back to the physician by our respiratory therapist.
